Medical care covered by insurance or Medicare continues to be billed separately according to your insurance benefits and applicable law. The membership fee covers only non-covered concierge amenities.

Family Member Add-On
- Additional Adult Family Member: 20% discount off the applicable membership rate
- Eligible Dependent (Ages 15–25): $100/month per dependent with Signature Plan amenities
- Available only for immediate household family members under the enrolled primary member
- Extends the same concierge membership amenities as the enrolled member’s plan
- Adult family members maintain their own physician-patient relationship, private account, and medical records
- Dependents who reach age 26 must enroll under their own individual membership to continue membership benefits
- Each enrolled family member is subject to physician acceptance and panel availability
How Membership Works with Your Insurance
The membership fee covers only concierge amenities and non-covered services. None of the benefits listed above is intended to be a service covered by your insurance plan or Medicare. Membership amenities are administrative and convenience services and do not change the covered medical care available to you or your physician’s clinical judgment. Medically necessary and covered care is provided to all patients based on clinical need and applicable law, regardless of membership status.
